How can I schedule an appointment?

Simply call (414) 443-0200 to schedule your appointment. Our staff will be happy to assist you, including finding a Health Dynamics provider near you! Or contact us via online form (link to contact page, online form).

Do I need a referral from my primary doctor?

No. A referral isn’t required for preventive care. We simply need to gather some basic information about your insurance plan to make sure you have a plan that adequately covers the Health Dynamics Program. All follow-up care — further testing, prescribing, treatments that may be recommended — will be performed by your primary doctor.

Is my doctor involved?

Yes. At your request and with your approval, a copy of your Personal Health Report can be provided for your physician. If any follow-up care is needed after your Health Dynamics exam, it will be handled by your doctor.

How do I get started?

Getting started with us couldn’t be easier! Simply call our office at (414) 443-0200. We’ll gather some insurance information, answer any questions you may have and schedule an appointment for you. Once we have received approval from your insurance plan, we’ll mail you information to prepare you for your exam, including our Health Assessment Questionnaire and instructions about what to expect, the exam duration, what to wear and bring and what you need to do prior to your exam.

Is this part of my insurance coverage?

Yes!. Most insurance plans now fully cover Preventive Care or an Annual Exam. We’ll preauthorize your coverage to make sure you’re eligible and to eliminate any surprises or out-of-pocket expenses. The Health Dynamics program is different in that we don’t diagnose, prescribe medications or treat conditions. These are things that usually trigger a change in billing codes that would otherwise cover Preventive Care, changing them to diagnostic or treatment codes that come with out-of-pocket expenses such as co-pays or deductible amounts. Here, it’s all covered, and there are no surprises.

What do I need to qualify?

To be eligible for Health Dynamics’ Preventive Care Program, you need to be over age 18 and be covered by a qualified health insurance plan from Aetna, United Health Care, Blue Cross Blue Shield/Anthem, Humana or WEA Trust. We’ll check with your insurance company to make sure you’re covered. Please keep in mind that we do not prescribe medications or treat any conditions through the Health Dynamics Program, so you will be advised to visit or call your primary doctor if you typically need diagnostic testing to follow up on existing medical conditions or if you are taking medications and need immediate refill prescriptions.

What happens during my first visit?

Once you’re here, we’ll welcome you to our office and set you up with a secure locker to store any belongings you would rather not carry through the assessment process. Our staff will then review your Health Assessment Questionnaire, ask if you are ready to provide a urine sample, perform a blood draw and guide you through the rest of your assessment, including the physician-directed exam. Once all components are complete, we will schedule a follow-up consultation (in-person or by phone) to review your exam results, answer any questions you may have and provide guidance for suggested health improvements.

What should I bring to my appointment?

Bring comfortable, loose-fitting clothes, athletic foot wear, any medications you may need, a small snack to eat after your lab work, your completed Health Assessment Questionnaire and the colorectal screening cards. You are welcome to review our Consent Form in advance of your appointment.

Is testing really at no cost to me?

Yes, it’s true. Because of the Accountable Care Act (ACA), and because we specialize in Preventive Care, we’re able to guarantee that people with qualified insurance coverage will incur no out-of-pocket costs. This no-cost aspect has proven difficult for most primary care doctors to accomplish, because they typically diagnose and treat conditions that may be discovered during the exam, changing the way the insurance plan views that “preventive care” visit. When they see a prescription, diagnosis or treatment billing code associated with a visit, the insurance company automatically applies and charges the patient with the co-pay and/or deductible amounts due. Health Dynamics has been very successful in saving patients from these unnecessary and unexpected out-of-pocket surprises.

What is included in my testing?

At Health Dynamics, most people claim that we’ve provided them the most comprehensive preventive care exam they’ve ever had, which includes:

  • Comprehensive health history questionnaire
  • Physician-directed physical exam
  • 43-point blood chemistry analysis
  • 16-point urine analysis
  • Blood pressure measurement
  • Electrocardiogram (EKG)
  • Cardiovascular fitness test (bike or treadmill)
  • Pap smear & screening (optional)
  • Screening mammogram (women)
  • Prostate cancer screening (men)
  • Colorectal cancer screening
  • Strength & flexibility assessment
  • Analysis of body-fat percentage
  • Pulmonary function (lung capacity) testing
  • Stress inventory

Do I have to be present for my counseling session?

No. You can request that your counseling session be conducted via telephone, if that is preferred or necessary.

However, we recommend that your consultation occur in-person with one of our expert Health Educators. But we certainly understand if a return visit is inconvenient or not practical, and we’ll gladly mail your Personal Health Report to you prior to the scheduled phone consultation.

Are same-day appointments available?

Same-day appointments are sometimes available, but we recommend that you plan ahead so we can provide the best service for all of our patients and make sure that you receive the full array of services, which is only possible with more advanced preparation. If you decide to request a same-day appointment, please call us first thing in the morning (7:00 to 7:30 am) to check on our schedule flexibility for that day. We’ll do our best to accommodate your request, but may not always be able to do so. And, again, we recommend advanced preparation so that you get the full scope and value of our Preventive Care program.
